Crinetics' PALSONIFY shows lasting benefits in acromegaly at ENDO 2026

Crinetics Pharmaceuticals has shared new long-term data for PALSONIFY (paltusotine) in acromegaly at ENDO 2026. The findings cover multiple trials, including PATHFNDR-1 and PATHFNDR-2, as well as combination therapy with oral cabergoline. The results highlight the treatment’s effectiveness and safety over extended periods. In the PATHFNDR-1 trial, patients switched from monthly injectable somatostatin receptor ligands to PALSONIFY showed stable IGF-1 levels at 0.81 times the Upper Limit of Normal after 96 weeks. Symptoms and pituitary tumour volumes also remained unchanged at 48 and 96 weeks.

The PATHFNDR-2 study reported a reduction in IGF-1 levels from 1.64 times ULN at baseline to 0.96 times ULN after 72 weeks of treatment. Additionally, pituitary tumour volume decreased by more than 20% in seven of 83 patients after 24 weeks. The therapy proved effective and well-tolerated in untreated acromegaly patients during the open-label extension.

A pooled analysis of 167 patients in the open-label extension trials found no new safety concerns. The most frequent side effects included diarrhoea, joint pain, headaches, and urinary tract infections. Combination therapy with PALSONIFY and oral cabergoline was also well tolerated.

At ENDO 2026, further data from the ACROBAT Advance study showed that adding oral cabergoline to PALSONIFY improved IGF-1 levels in some patients. The long-term data confirm PALSONIFY’s sustained effectiveness and tolerability in acromegaly. IGF-1 levels stabilised or improved across trials, and tumour volumes reduced in some cases. The combination with cabergoline offers an additional option for patients needing further control.
